This analysis aims to characterize the urinary tract infections (UTI) that occurred in all patients of Shirley Ryan AbilityLab (SRAlab), an acute inpatient rehabilitation hospital in Chicago, with the goal of identifying future intervention targets to reduce UTI incidence and reduce the phenomenon of urine overculturing. The results of this analysis will be used to design UTI-prevention interventions directed toward caregivers and patients, and strategies to prevent over-culturing and maximize resources, improving patient outcomes.

- Prevalence and Impact of Adverse Childhood Experiences in the Hispanic Community by Questionnaire Language Preference: Behavioral Risk Factor Surveillance System (BRFSS) 2011, 2012
- Abstract:
- Objective: The study analyzes the prevalence of Adverse Childhood Experiences (ACEs) in the Hispanic population of nine states, using 2011-2012 Behavioral Risk Factor Surveillance System (BRFSS). The study compares ACEs prevalence and adult self-reported health status, chronic conditions, and behavioral risk factors by English versus Spanish survey language.Methods:. BRFSS data are weighted estimates that reflect state level populations. ACEs were compared by questionnaire language preference and by respondent sociodemographic characteristics. Self-reported health status, behavioral risk factors and chronic conditions were dichotomized into adverse adult health outcomes and compared across four categories of ACEs score (0, 1, 2-3, 4+). Chi square tests were used to determine the statistical significance of language preference differences in individual ACE items and sociodemographic characteristics. Multiple logistic regression, adjusted for differences in language preference within each level of ACEs score, was used to analyze the likelihood of adverse adult health outcomes independently associated with language preference across all ACE score categories. Results: From over 68,000 respondents 2,983 affirmed being Hispanic or Latino, representing a weighted population of 1,855,212. For the overall cohort and both language preference groups, respondents ages 35 to 49, women, and the uninsured reported the highest percentage of 4+ ACEs. Verbal abuse was the most common adverse childhood experience reported (27.2%) while being forced to have sex was the least common (4.9%). The prevalence of the majority of individual adverse childhood experiences differed significantly between the English and Spanish questionnaire preference groups (p<0.01). Reporting 4+ ACEs was associated with an elevated prevalence of disability, cancer, COPD, asthma, depression, smoking, and heavy drinking but not diabetes, self-reported, physical, mental, and general health, and in the overall cohort. Differences between the English and Spanish 4+ ACE groups included higher odds of having cancer, having smoked, and being a heavy drinking among English preference respondents and higher odds of poor or fair general health, cardiovascular disease and COPD among those that preferred Spanish.Conclusions: There were substantial differences in both ACEs prevalence by BRFSS language preference and in both populations there was a strong association between effects of ACEs score on adult health outcomes. The superior health status of Spanish language respondents, a younger, healthier population, may in part correspond to differences in immigrant health, or as described in terms of cultural differences, acculturation. However, it is important to note that the nine states studied reflect smaller, more isolated Hispanic communities than those of high Hispanic population states usually studied. The greatest association between high ACEs score and bad health outcomes was seen among English survey language survey respondents with the highest ACE score. Public health and medical professionals are trying to address childhood adversity as we better understand how it is a root cause for many negative adult health conditions. Approaches to universal screening by pediatricians and trauma informed care for adults are evolving.

- Assessing Barriers for Fertility Preservation Among Transgender Youth: Insurance, Costs, and State-Specific Policies
- Abstract:
- Increasingly, children, adolescents and young adults with a diagnosis of gender dysphoria are presenting for care desiring medical interventions to align their bodies and gender identities. For patients for whom pubertal development has progressed to Tanner stage 4 or 5, interventions include treatment with testosterone for birth-assigned males and estrogen for birth-assigned females. For younger children, puberty blockers can delay unwanted physical maturation until a decision is made about either stopping blockers and progressing through their endogenous puberty or initiating gender-affirming hormone treatment. These gender-affirming treatments may compromise gonadal function leading to infertility or biological sterility. Fertility preservation (egg and sperm freezing) prior to gender-affirming hormone treatment offers youth pursuing these treatments options for future biological parenthood. This paper begins with an overview of currently available medical treatments and the costs for procedures in this unique population. Subsequently, a review of both state and federal insurance coverage policies regarding fertility preservation and other fertility care procedures is offered, focusing specifically on the case studies of Illinois, Connecticut, and Rhode Island where recent changes in state insurance codes have expanded coverage for these medical services. Finally, the paper provides an analysis and discussion of relevant qualitative interview data from the Trans Youth Fertility Study (TYFS), an ongoing project using qualitative interviews with transgender youth and parents to investigate their experiences with navigating medical insurance and costs associated with fertility preservation.

- Racial Disparities in Non-Traumatic Lower Extremity Amputation Rates in Illinois, 2013-2016
- Abstract:
- Background. Racial disparities in amputation rates have been widely recognized, including studies of higher rates in the Chicago metropolitan area among predominantly non-Hispanic black as compared to predominantly non-Hispanic white zip code areas. These disparities persisted into the 2000s despite a declining overall amputation rate after 1996. This study compares racial and ethnic amputation rates from more recent years for all residents of Illinois.Methods. Illinois hospital discharge data from 154 non-federal Illinois hospitals were used to calculate lower extremity amputation rates for the years 2013-2016 by race and ethnicity. ACS 5-year Census data were utilized to create race and ethnicity population denominators for patients 35 years of age or older in the state of Illinois, as well as for the populations of 3 different groups being compared (non-Hispanic white, non-Hispanic black, and Hispanic or Latino). The study assessed statewide amputation procedure trends as well as trends in population-based differences in through foot, below knee, and above knee amputation rates per 100,000.Results. Between 2010 and 2014, there was an apparent 14.7% increase in total lower extremity amputations in National Inpatient Sample estimates. In Illinois, statewide amputation procedure rates at all levels increased steadily from 2013 to 2016, increasing to 38.29 per 100,000 in 2016. However, there were very apparent ICD-9 to ICD-10 coding effects, with a 13% jump in through foot amputations and a 10% decline in above knee procedures between quarters when coding transitioned in 2015. Though only representing approximately 13% of the Illinois population, 26.9% of all lower extremity procedures were performed for non-Hispanic blacks, with the disparity in overall rates averaging 48.5 per 100,000, twice the non-Hispanic white rate.Conclusions. There was an increase nationally in the number of amputation procedures preceding the change to ICD-10 coded procedures in 2015. These data from Illinois hospitals indicate an apparent continuing increase in amputations driven by a large 2016 increase in through foot procedures, that is to some extent an IVC-10 coding change artifact. While ICD-10 coded above knee amputations appear to have declined in 2016, we cannot determine whether this was a real or a coding-related decline. Racial disparities in lower extremity amputation rates in Illinois continue to exist. Compared to a prior study looking at amputation rates in Northern Illinois, it possible that the racial gap has narrowed since the early 2000s, suggesting that public health interventions involving education and addressing social determinants of health may be promising to continue to reduce the gap.

- The Fate of Obstetric Fistula: Towards Equity, Education, and Policy in the Sylhet Region of Bangladesh
- Abstract:
- This project will describe a review of literature and recommendations for obstetric fistula prevention programming and policy in the Sylhet region of Bangladesh. In order to better understand the social implications that women suffering from obstetric fistula face, an analysis of the interpersonal health communication topics of stigma, disclosure, coping, and identity will be conducted. Obstetric fistulas are an issue of health equity for women in resource-poor settings across the globe. An obstetric fistula is a hole that forms between the vagina and rectum or bladder as a result of prolonged obstructed labor, leaving a woman incontinent of urine, feces, or both. It is estimated that more than two million women currently live with untreated obstetric fistulas in sub-Saharan Africa and Asia (countries with the highest prevalence). The Sylhet region of Bangladesh has been disproportionately impacted in comparison to other regions. There are currently 70,000 women in need of fistula repair surgery and approximately 2,000 new cases of fistula develop every year. Only about 40% of births are attended by skilled personnel and the lifetime risk of maternal death is approximately 1 in 240. Programming recommendations include raising awareness on fistula prevention by training skilled personnel on the signs/symptoms and treatment options. Training community health workers on access, signs, and symptoms of obstructed labor provides an opportunity for depth and breadth of community-based prevention efforts. Training lay individuals to become skilled birth attendants is essential given the lack of midwives and health professionals in rural areas. Health and education policy should focus on training and access for all women at national and regional levels.

- Classification and Regression Tree (CART) analysis to predict successful delivery of a YMCA childhood obesity intervention program
- Abstract:
- Background: Obesity and its co-morbidities account for a large proportion of chronic disease in the U.S. In an effort to reduce rates of obesity among children, YMCA of the USA (Y-USA) has developed a program called Healthy Weight and Your Child (HWYC), a multi-session intervention program for children carrying excess weight aged 7-13 years old delivered through local YMCAs. To date, 19 local YMCAs have successfully delivered pilot versions of the program. As Y-USA moves from the validation stage of HWYC to translating and scaling the program, Y-USA hopes to improve the model by identifying factors in program delivery that best lead to successful delivery of the program. This analysis used classification and regression tree (CART) modeling to identify such explanatory factors that predict successful delivery of HWYC in local YMCAs, as measured by Change in child BMI and % Attendance.Methods: Participants in HWYC pilot programs included children with obesity (BMI > 95th percentile for age and gender) (n = 782). Data collection included baseline and session 20 weight, height, and waist circumference measures, as well as demographic information. Attendance was measured at every program session. Bivariate and CART analysis was performed in R using previously collected and de-identified data.Results: CART analysis found that a participants attendance is most predicted by the YMCA site delivering HWYC, as well as demographic factors such as race and eligibility for free/reduced lunch. The baseline BMI of the accompanying adult also influenced attendance. CART also found that class size was a major predictor of change in childs BMI, as were program model (yearlong vs. condensed) and sex.Conclusions: The CART analysis successfully identified several factors 40 that predict both attendance and childs BMI change among participants in the HWYC program. Results suggest that Y-USA should scale HWYC to more YMCAs using the condensed 4-month program model to achieve greater successful outcomes. Additionally, individual YMCAs may need to consider the demographics of their unique communities in order to improve both attendance and BMI reduction. As the sample size of the condensed program was small (n = 120), future studies are required to better understand unique explanatory factors that predict successful delivery of HWYC under this program model.

- Barriers and facilitators to cervical cancer screening among Asian American women: a systematic review
- Abstract:
- Objective: To understand barriers and facilitators for obtaining cervical cancer screening in Asian American women age > 18 years.Data Sources: A systematic review following PRISMA guidelines and using PubMed, EMBASE, and Web of Science to identify studies. Search terms included cervical cancer screening, Asian American, and Pap test. Additional studies identified manually.Study Selection: Only articles assessing sociodemographic, healthcare access, or acculturation factors and knowledge, attitudes, and behaviors in Asian American women associated with compliant and noncompliant cervical cancer screening behavior were included.Data Extraction: Extraction of data by 1 author using predefined data fields.Data Synthesis: A total of 17 articles met all inclusion criteria. Six groups were studied: Chinese, Filipina, Japanese, Korean, South Asian, and Vietnamese. Age, marital status, education, insurance coverage, time in the US, and English-speaking ability were associated with previous receipt of a Pap test. Young age, unmarried, uninsured, less time in the US and less than fluent English-speaking ability were less likely to have obtained a Pap test. Older age, married, private or public health insurance, more than ten years in the US, and fluent English-speaking ability were more likely to have obtained a Pap test. Variations of this trend were found among the subgroups. Not enough data on knowledge, attitudes, and behaviors to make generalizable statements.Interventions: Multi-component, community-based interventions using patient navigators, text messages, or lay health workers have varying degrees of success in increasing uptake of Pap tests but can bridge gaps in care and overcome barriers faced by Asian women seeking cervical cancer screenings.Limitations: Only applicable to the populations discussed. Not applicable to the entire Asian American community or other minority populations.Conclusion: Successful interventions to increase use of cervical cancer screenings must be targeted in their approach and demonstrate a thorough understanding of the characteristics of individual communities.

- Evaluation of Adolescent HIV Surveillance and Testing Program in Uptown Chicago, 2013-2017
- Abstract:
- Introduction: Adolescent Men who have sex with men (MSM) and transgender women (TW) are currently overrepresented among new HIV cases in the United States. To address this concern, the Adolescent Medicine division of Lurie Children's Hospital has operated an HIV testing program in Uptown Chicago geared towards these individuals. The purpose of this manuscript is to report on the evaluation of the HIV outcome, demographic, and risk behavior information resulting from longitudinal data collection performed as part of this program. The results of this paper will be used to drive future program direction and improve primary and secondary HIV prevention among LGBTQ youth in Chicago.Methods: From May 2013 to June 2017, 1058 individuals presented for HIV testing in the Adolescent Medicine division and filled out questionnaires detailing demographics, sex behavior and known HIV risk factors. This data was aggregated in R and subsetted by gender and sexual orientation. Bivariate analysis was conducted to compare risk factors among HIV positive and negative MSM and TW as well as compare old MSM to adolescent MSM HIV positive individuals. A logistic regression was performed to analyze risk factors for a positive HIV test among young MSM.Results: For MSM and TW, those of African American race were at the highest risk for a positive HIV test. In young MSM, social factors such as homelessness and previous incarceration were more salient risk factors than sexual behavior such as condomless sex. The TW tested showed a significantly higher HIV risk than the young MSM tested, but did not show significant differences between positive and negative HIV results. Older MSM individuals were more likely to report having condomless sex with a male or sex with an HIVpositive male partner. Conversely, individuals in the young group were more likely to report having sex with multiple partners and report having sex for drugs or money.Conclusion: The data presented in this analysis mirrors a trend in the United States; while HIV total infections are decreasing, African American youths show an increasing risk. The results also show the primary difference between those who receive a positive HIV test result and those who receive a negative one are social in nature and not precisely related to sex behavior. The Adolescent Medicine division can use these results to direct their resources towards primary prevention of HIV infection in young MSM and TW.

- Recommendations for How Illinois's Medicaid Managed Care System Can Positively Impact Care for Children with Special Healthcare Needs
- Abstract:
- Introduction: Illinois aims to introduce 29,000 Children with Special Healthcare Needs (CSHCN) into its Medicaid Managed Care (MMC) program starting in 2018. In theory, the care coordination and quality-based incentives in MMC could improve care and reduce healthcare costs for this population. However, other states have experienced issues bringing such complex patients into a system that aims to be streamlined and cost-effective. Due to the large medical costs and complexities of CSHCN, improving care for this population in MMC could lead to a significant number of pediatric health improvements and cost savings across Medicaid. This paper aims to draw on experiences from previous implementations and provide recommendations for states considering this transition, specifically around what requirements to include in insurer contracts and how to ensure insurers are meeting their obligations.Methods: Five areas of focus were selected: reduction of racial and income disparities, ability to monitor and ensure network adequacy, ease of transition into and between managed care providers, adequacy of provider payments, and protection of EPSDT benefits. In each area, a literature review was conducted to identify best practices among states who previously went through this transition. These strategies were separated into recommendations for insurer contracts and recommendations for implementation, and apply to all states who are still looking to integrate CSHCN into MMC. These recommendations were then compared to the Illinois model contract with insurers, to create suggestions specific to Illinois.Results: Contract and implementation recommendations for all states, as well as specific suggestions for Illinois, were provided for each focus area. In each focus area, Illinois had already integrated at least one of the reviews recommendations, but was also missing at least one.Conclusion: Across all focus areas, there were three principal recommendations for states looking to introduce CSHCN to MMC. States would ideally provide CSHCN-specific exceptions in MMC contracts when necessary, attempt to align insurer and state goals through incentive updates, and employ comprehensive monitoring systems to oversee insurer behaviors. These recommendations can be used as a platform for discussion between organizations within the Child Health Policy Collaborative to help shape future research and strategies of the collaborative.

- Epidemiology of Cancer Survivorship among Young and Emerging Adults
- Abstract:
- Background: Each year in the United States, an estimated 70,000 incident cancer cases are diagnosed among the young and emerging adult (YEA) population, and cancer is one of the top five leading causes of death in this age group. The YEA cancer population consists of those diagnosed with cancer between ages 18-39. YEA cancer patients present with unique risk factors for late effects of cancer treatment due to their transitional life status as they are undergoing social, emotional, and developmental transitions during the period of their cancer diagnosis, treatment, and survivorship. The goal of this study is to better understand the unique epidemiological factors in this population to improve care recommendations.Methods: Data was collected using the Northwestern Enterprise Data Warehouse (EDW), which contains medical records on YEA cancer survivors treated in the Northwestern Hospital system. Participants were eligible if they were diagnosed with any cancer in the Northwestern Hospital system between ages 18-39 and were diagnosed between 1/1/2000 and 1/1/2019. The dataset contained information for 14,552 patients including the patients study ID, gender, ethnicity, race, cancer diagnosis date, cancer type, age at diagnosis, secondary cancer, date of death, cerebrovascular disease, coronary artery disease, diabetes, heart failure, hyperlipidia, hypertension, peripheral arterial disease, and renal failure. Frequency analyses were used to determine distribution of each cancer type in the cohort and each cancer type by gender. Mann Whitney U test was used to determine mean age at diagnosis for each cancer type. Frequency distribution was used to determine racial and ethnic demographic distribution among the five most common cancers. Logistic regression was used to determine odds of developing comorbidities (heart failure, hyperlipidia, hypertension, secondary cancer). Survival analyses were conducted using Kaplan-Meier Survival Curves and Cox Proportional Hazards Models for breast cancer and lymphoma patients and time to hypertension diagnosis.Results: The most prevalent cancers in this population are breast, lymphoma, thyroid, melanoma, and brain and nervous system. Compared to non-breast YEA cancer survivors, breast cancer patients had a significantly higher odds of a secondary cancer diagnosis (OR= 2.4, 95% CI= 2.2-2.6). Compared to non-lymphoma YEA cancer survivors, lymphoma patients had a significantly higher odds of heart failure diagnosis (OR= 2.2, 95% CI= 1.7-3.0), hyperlipidia diagnosis (OR= 1.6, 95% CI= 1.4-1.9), and hypertension diagnosis (OR= 1.2, 95% CI= 1.1-1.4). Thyroid cancer patients had a significantly higher odds of a secondary cancer diagnosis (OR= 1.4, 95% CI= 1.2-1.5) compared to non-thyroid YEA cancer survivors. Kaplan-Meier estimates showed that race and secondary cancer status may play a role in time to hypertension diagnosis in YEA breast cancer patients. However, in Cox Proportional Hazards model, race and secondary cancer status had no impact on time to hypertension diagnosis. For lymphoma patients, Kaplan-Meier estimates showed that gender, race, and secondary cancer status may play a role in time to hypertension diagnosis. However, Cox Proportional Hazards model showed no impact of these variables on time to hypertension diagnosis.Conclusions: This study provides important public health implications by identifying the most prevalent cancers, mean age at diagnosis, and risk of future comorbidities among YEA cancer survivors in the Northwestern Medicine health system. It also provides evidence for possible factors impacting survival in this population.

- Integrated Reproductive Healthcare in School-Based Health Centers Improves Adolescents Access to Long-Acting Reversible Contraception
- Abstract:
- Objective: Long-acting reversible contraception (LARC) is the most effective reversible method of contraception, yet there remain many barriers preventing adolescents in the United States from using it. To mitigate these barriers, Heartland Health Center (HHC) changed the workflow in their school based health centers (SBHCs), introducing a novel integrated model of reproductive healthcare. This project aimed to assess changes in contraceptive provision based on the new model of care. Methods: The integrated model of care includes a certified nurse midwife (CNM) staffing the SBHCs intermittently to provide longer counseling sessions and dedicated time for procedures. Electronic medical records were utilized from before and after the implementation of this model to assess change. Chi-square tests were used to test the significance of change and the association between clinical and demographic data. A Poisson regression model was utilized to estimate the incidence rate ratio (IRR) for the likelihood of receiving LARC.Results: LARC prescriptions rose from just 0.6% in 2016 to 3.9% in 2018 (p<0.001). Further, data showed that students were 17.4 times more likely to get LARC prescriptions in 2018 than in 2016 (p<0.001). The percentage of prescriptions for emergency contraception rose from 9.8% to 24.1% and the percentage of prescriptions for injections fell from 43.7% in 2016 to just 27.1% in 2018.Conclusions: The present model of integrated reproductive healthcare not only has the potential to improve access to methods of contraception that are challenging to receive, but it also provides an opportunity for specialized providers to deliver culturally competent, trauma informed sexual health education and medical care. This study also provides the foundation for a future study investigating student experience and outcomes based on the new model.

- Effect of Genetic and Lifestyle Risk Factors on the Association Between Body Mass Index and Personal History of Breast and Ovarian Cancer in the Bright Pink Population
- Abstract:
- Objective: To investigate the effect modification by genetic and potential lifestyle risk factors on the association between BMI and breast and ovarian cancers diagnosed among women 20-40 years of age.Methods: We conducted a case-control analysis with 1511 breast cancer cases and 6044 controls and 1267 ovarian cancer cases and 5068 controls between 20-40 years of age. The study population and data were obtained from the Bright Pink, a national non-profit organization dedicated to the prevention and early detection of breast and ovarian cancer in young women. Cases and controls were frequency matched by age category. The association between BMI and Breast and ovarian cancers were modelled separately using logistic regression. Effect modification by hereditary gene mutations and behavioral factors including physical activity, cigarette smoking, alcohol consumption and oral contraceptive use were evaluated in each of the final models at the 0.10 significance level.Results: Compared to the normal weight group, the underweight group had more breast cancer diagnoses (OR adjusted 95% CI=1.31 [1.05, 1.64]) while the obese group had less (OR adjusted 95% CI=0.82 [0.70, 0.96]). Only women the underweight group were significantly more likely to have a personal history of ovarian cancer compared to the normal weight group (OR adjusted 95% CI=1.69 [1.32, 2.15]). Physical activity, alcohol consumption, and smoking were found to be effect modifiers in the associations between BMI and breast and ovarian cancers. Being underweight was related to increased personal history of breast and ovarian cancers among women who exercised regularly, were current smokers, or had >1 drink per day while the magnitude of association was attenuated among women who did not engage in these health behaviors.Discussion: Our study supports recent findings of BMI as a protective factor for premenopausal breast cancer in literature. Interaction between BMI and several behavioral factors suggest young, underweight women may have higher susceptibility to carcinogenic effects of environmental exposures in breast and ovarian cancer development. The results of this case-control analysis are subject to potential recall bias and reverse causality. Future prospective studies are needed to verify these findings.

- Seasonal and gendered variations in experiences and perceptions of water insecurity among smallholder farmers in western Kenya: a qualitative study
- Abstract:
- Water insecurity contributes to negative physical and mental health outcomes, and has been found to disproportionately affect women, who are typically primarily responsible for fetching water and managing its use. However, little is understood about how water insecurity manifests among agrarian, HIV-affected populations in sub-Saharan Africa. To explore water insecurity experiences among HIV-affected, smallholder farmers in western Kenya, the unique experiences of women in this context, and coping strategies to water insecurity consequences, data were collected as part of the Shamba Maisha study (NCT02815579). The Shamba Maisha study tests an agricultural intervention to improve the health and nutrition of people living with HIV. Interviews were conducted with 30 adults (24 women; 6 men; age = 41.79.4 years). Interviewers asked open-ended questions, probing about water acquisition, uses of water, challenges, strategies to cope with water-related challenges, farming practices, and perceived benefits of the Shamba Maisha program through the go-along interview method. A coding framework was developed after identifying emergent themes, and analysis was conducted using a combination of inductive and a priori methods using Atlas.ti 8. Almost 70% of interviewed households experienced water scarcity and poor water quality. Water insecurity challenges and associated consequences varied depending on the season. Women were responsible for the acquisition and management of water (90%), which resulted from normalized gender roles. Primary methods of coping with water insecurity were storing water (96.7%), treating water (86.7%), and diversifying water sources (83.3%). Shamba Maisha study participation increased participants empowerment, offered new agricultural opportunities, and increased hope for the future. Addressing water insecurity must be specific to the season and differences in water experiences across gender, and has the potential to significantly improve physical and mental health outcomes of the women of the household because of their responsibility over water use and management.

- Trends in Gun Shot Wound Related Emergency Department Visit in Illinois
- Abstract:
- Introduction: Gun violence is the lowest its been in the past 50 years across the US. However, the number of circulating guns is the highest its been, and gun violence endemics continue to exist in high crime cities like Chicago, with 2016 having one of the highest homicide rates in the citys history. Historically, gun crime was attributed to demographic risk factors, but recent research has suggested that social networks play a significant role in propagating gun violence and may serve as a crucial point of intervention.Study Aims: The study aims to utilize a novel hospital-based database to track the gun violence-associated burden on the healthcare system. Methods: The Illinois Hospital Association Health Care and Hospital Data Reporting Services (COMPdata) data for 159 non-federal hospitals was used to analyze trends in ED Gunshot Wound visits over a 27-month period. A Poisson regression was performed to identify Incidence rate ratio (IRR) of mortality and inpatient hospital admission from GSW-related ED visits. Results: The summer of 2016 recorded the greatest number of GSW-related ED visits, with August of 2016 having 389 visits. Overall, assault GSW accounted for 89.1% of all visits, with Cook County accounting for the greatest number of ED visits. There was no significant difference in mortality by patient groups, however, Hispanics and Medicaid/Medicare patient had significantly higher inpatient admission IRR. Discussion: Gun violence in Illinois is mostly attributable to spike and drops within the city of Chicago. Hospital data are one approach to track gun violence within the state of IL, and the ED can provide an important venue for addressing social risk factors associated with gun violence.

- Contraceptive Justice Project at EverThrive Illinois: Understanding the Patient/Client Perspective through Focus Groups
- Abstract:
- Introduction: EverThrive Illinois has been working alongside partners and members to identify and address systemic causes of health inequities in Illinois affecting women, children, and families, through lasting policy changes. Recently, they initiated the Contraceptive Justice Project to increase access to all forms of contraception, while considering the impact of ten social determinants of health. As part of the patient/client assessment, a literature review was conducted that revealed common facilitators and barriers to contraceptive access, and major themes related to a number of social determinants of health. The literature review also revealed gaps in data regarding access to contraception for immigrant populations and the transgender community.Methods/Analysis: Four focus groups (N=57) were conducted at various community organizations in Chicago in order to address the gaps in the literature. Focus group notes were analyzed using grounded theory and an iterative process of comparison. A de-identified, open-ended, demographics questionnaire was administered and analyzed using descriptive statistics.Results: Sample consisted of presumed ciswomen (79%) and gender-expansive folks (12%) who self-identified as Latinx (51%), Black (16%), and white (21%). Four major themes emerged from the focus group discussions: sexual and reproductive health, sexual health education, physician experience, and contraception experience. Discussions revealed multiple sources of sexual and reproductive health information and that experiences of trauma were common. Sexual health education yielded positive experiences when curriculum was inclusive, comprehensive, and taught in multiple modalities. Patient-physician experiences were influenced by language, feelings of distrust, and confidentiality. Finally, when discussing contraceptive experiences, participants often mentioned side effects and cited several barriers to access to their preferred method including insurance/cost, providers pushing contraception methods, or being offered limited options.Discussion: In order to increase access to preferred methods of contraception, people seeking contraception must have access to comprehensive, inclusive, trauma-informed, and age-appropriate sexual health education. The patient-physician relationship also must be strengthened by improving communication, using understandable and inclusive language, and respecting people and their choices. Finally, contraception counseling should be RJ informed.

- Arthritis Prevalence and Disability Among Young to Early Middle Age Adults in the US: Findings from the 2017 Behavioral Risk Factor Surveillance System
- Abstract:
- Objective: This study profiles the prevalence of an arthritis diagnosis by key clinical and sociodemographic characteristics of the adult U.S. population age 18-55.Methods: The study included 195,822 respondents which represent an estimated 136.7 million residents of the United States, Guam, and Puerto Rico. The 2017 Behavioral Risk Factor Surveillance System (BFRSS) data was used to compare self-reported physical activity, mental health and physical health between respondents with and without an arthritis diagnosis within three age groups, young adults (age 18-34 years), older young adults (age 35-44 years), and early middle age adults (age 45-54 years). We used logistic regression to test the significance of social, demographic and clinical factors associated with the likelihood of an arthritis diagnosis under age 55. We then compare young to early middle age adults who reported being diagnosed with arthritis by three levels of reported pain and by the degree to which they perceive that arthritis limits their functional, social and work lives. Results: Across all three age groups, adults with arthritis reported increased inactivity and poorer mental and physical health compared to age-matched controls without arthritis. Being diagnosed with arthritis before age 55 was strongly associated with having ever been diagnosed with a depressive disorder (OR:3.10), being obese (OR:2.06), and reporting the lowest household income level as compared with the highest (<$15,000 versus >$75,000, OR:2.26). Among respondents under age 55 with arthritis, being female, obese, reporting poor mental health, lower socioeconomic status, and minority race and ethnicity were all significantly associated with higher reported pain levels and greater arthritis-related functional, social, and work limitations.Conclusion: Arthritis prevalence and disability among adults under age 55 follows a steep social class gradient that is closely associated with mental health and obesity. These findings can guide targeted primary and secondary preventive interventions to encourage healthy weight and support mental health, with the goal of limiting the impact of the disease.

- Abstract:
- Erected in 1769 on a project by the architect Francesco Collecini (1723-1804) on the existing "Torretta del Perness", the recently restored Castelluccia is an octagonal fort on three orders, protected by a moat, with streams and drawbridges.Born as a structure intended for the military exercises of little Ferdinando IV, the building was transformed in the nineteenth century into a place of delights and amusements: the castle became a pleasant entertainment site, embellished with a belvedere and a garden, while the barracks surrounding areas have become pavilions for cooking, lunch and coffee.In a corner of the garden there is a "Chinese dome", made according to the fashion of the time, while in the part in front of the Castelluccia you can still admire splendid specimens of magnolias, araucarias and palms, planted at the time to decorate the place.
