- Abstract:
- The complication of thrombotic thrombocytopenic purpura or hemolytic uremic syndrome (TTP/HUS) can occur in cancer patients. It is characterized by a microangiopathic hemolytic anemia, severe thrombocytopenia, and renal failure. Pulmonary manifestations, especially pulmonary edema, are a common observation. Neurologic changes are also frequently seen. The etiology is unknown at this time. It has been observed in many different types of cancer and is most commonly seen in gastric adeno-carcinoma followed by carcinoma of the breast, colon, and small cell lung carcinoma. The hemolysis can be massive and is due to red cell fragmentation, as schistocytes are present in all the cases. Though immune complexes are present in the plasma, the antiglobulin (Coomb's) test is negative. Chemotherapeutic agents, especially mitomycin C, have been implicated as causative factors. There is a correlation of this complication with the cumulative dose. However, chemotherapy cannot account for all the cases as the syndrome can occur in untreated patients. It can be differentiated from disseminated intravascular coagulation by the absence of a coagulopathy. Management should consist of plasma exchange, use of a Staphyloccus aureus column (Prosorba), and control of hypertension. Because of the susceptibility to pulmonary edema, blood volume overloading should be avoided.

- Abstract:
- Thrombotic microangiopathy, manifesting as thrombotic thrombocytopenic purpura (TTP) or hemolytic uremic syndrome, is a common complication in cancer patients. It shares the pathogenic microvascular occlusive lesion and many clinical manifestations as the classical TTP, but the spectrum of complications varies widely. Several subsets are seen, including a microangiopathic hemolyticanemia in advanced cancer, chemotherapeutic drug-associated microangiopathy and those with the transplant setting. The prognosis is not as favorable as in classical TTP. Anecdotal reports indicate that responses are seen with plasma exchange and with immunoadsorption.

- Abstract:
- It is well recognized that cancer patients have an increased risk of thrombosis and bleeding. Contributory factors to thrombotic complications include prothrombotic risks carried by many chemotherapeutic agents, type of cancer, stage of cancer, age, and comorbidities. Surgical procedures and the presence of an indwelling vascular device, often used as a mode of delivery for chemotherapy, further increase the risk. Correlative studies have demonstrated upregulation of coagulation in response to chemotherapeutics, and clinical studies have shown that various cancer treatments are independent risk factors for thrombotic complications. It has also been shown that patients who develop thrombosis during treatment have a worse overall prognosis. Mechanisms for chemotherapy and cancer-associated thrombosis are not well understood. Better insight into the mechanism for thrombosis may help better identify those cancer patients at highest risk, who may then benefit from up-front anticoagulant prophylaxis.

- Abstract:
- The risk of both thromboembolic and bleeding complications is high in acute leukemia. This double hazard has a significant negative impact on the morbidity and mortality of patients with this disease. The clinical manifestations of both complications show special features specific to the form of acute leukemia. Recognition of these characteristics is important in the diagnosis and management of acute leukemia. In this article, several additional issues are addressed, including the features of bleeding and thrombosis in acute promyelocytic leukemia, the current understanding of the leukostasis syndrome and the iatrogenic complications including catheter-associated thrombosis, and the adverse effects of therapeutic agents used in acute leukemia. As regards the bleeding complications, thrombocytopenia is a major cause. Corrective measures, including recent guidelines on platelet transfusions, are provided.

- Abstract:
- The hemostatic abnormalities seen in acute promyelocytic leukemia (APL) are unique and account for much of the morbidity and mortality of this disorder. Almost all patients present at diagnosis with laboratory findings of intravascular coagulation along with increased fibrinolysis. This unusual combination is correlated to the clinical manifestations with high risk of both bleeding and thrombosis. Recent studies have revealed that the leukemic promyelocytes in APL express increased amounts of tissue factor as well as elements of the fibrinolytic system, including tissue plasminogen activator, annexin A2, and plasminogen activator inhibitor type 1. These changes are responsive to differentiation therapy with all-trans-retinoic acid (ATRA) or with arsenic trioxide (ATO). Despite a dramatic reduction in mortality seen since the introduction of differentiation therapy with ATRA or with ATO, a large number of deaths still occur before complete remission is achieved. The early deaths are mostly attributable to the presenting coagulopathy. The prevention and management of this hemostatic abnormality have thus far been unsuccessful and remain a challenge to bring about a higher cure rate for this disease.

- Abstract:
- The endothelium is recognized today as a functional and dynamic component of the body that plays an important part in health and disease. This article briefly reviews the role of endothelium in thrombosis and hemostasis. There is a paradigm shift from one that regards the endothelium as one single entity to the concept that the endothelium isheterogeneous. Thrombotic complications in many disorders show a predilection to specific locations, despite the fact that a hypercoagulable state affects the entire body. Likewise, bleeding is more commonly encountered in certain anatomical locations than in others. Recent observations of the heterogeneous distribution of coagulation and fibrinolytic factors in the endothelium and the adaptation of the endothelium to various stimuli may provide an explanation for the diverse phenotypes. Recognition of this changing paradigm is helpful to for the diagnosis and management of bleeding and thrombotic complications. It also helps in the understanding of the pathogenesis of many bleeding and thrombotic disorders, and in the development of new drug designs for site-specific therapy.

The "stars" here are salt crystals. These help solidify the hydrogel. By forming a flexible, jelly-like material, these hydrogels can be 3D-printed into any shape. For example, we can print hydrogel implants to heal damaged cartilage in human joints.

- Purpose: Because older adults are at elevated risk of COVID-19-related adverse health outcomes, and staying at home is an effective strategy to avoid unnecessary exposures, the current formative study used the Reasoned Action Approach (RAA) to identify the beliefs underlying older adults' decision to stay home for the next month. Methods: The participants (weighted n = 206, age 65-94) for the current study were selected from a nationally representative online survey of US adults from April 10-20, 2020. We used multiple linear regression to estimate the relative contribution of the four RAA global constructs (instrumental attitude, injunctive norms, descriptive norms, and self-efficacy) in explaining intention to stay home after controlling for demographic covariates. We also conducted a content analysis to identify beliefs about advantages, disadvantages, and facilitators of staying home. Results: After controlling for demographic characteristics, injunctive norms (b = 0.208; SE = 0.059; B = 0.213, p < 0.01) and self-efficacy (b = 0.532; SE = 0.058; B = 0.537, p < 0.001) showed statistically significant independent associations with intention to stay home. The specific beliefs underlying the decision to stay home spanned across health and wellness dimensions and suggested interpersonal, mental health, and leisure/recreational facilitators. Conclusions: These findings suggest three public health intervention targets. First, self-efficacy building interventions could enhance older adults' perceptions of their ability to stay home to avoid unnecessary exposures. Second, health communication messages to address injunctive norms could emphasize that people important to older adults think they should stay home. Third, for the youngest of the older adults, health communication messages could emphasize the advantages of staying home.

Compared with severe acute respiratory syndrome and Middle East Respiratory Syndrome, COVID-19 spreads more rapidly owing to increased globalization, a longer incubation period, and unobvious symptoms. As the coronavirus has the characteristics of strong transmission and weak telethality, and since the large-scale increase of infected people may overwhelm health care systems, efforts are needed to treat critical patients, track and manage the health status of residents, and isolate suspected patients. The application of emerging health technologies and digital practices in health care, such as artificial intelligence, telemedicine or telehealth, mobile health, big data, 5G, and the Internet of Things, have become powerful weapons to fight against the pandemic and provide strong support in pandemic prevention and control. Applications and evaluations of all of these technologies, practices, and health delivery services are highlighted in this study.

- The Role of Activism in Combating Feelings of Helplessness Among Chicago Adolescents: A Qualitative Analysis
- Abstract:
- Background: Racism is a public health issue that has both immediate effects and also long term, generational consequences. Racisms impacts spans from increased incidences of chronic physical health and mental health conditions to lost job opportunities and affordable housing. Racism is pervasive and has persistent effects on a significant portion of the US population. Previous research has explored the effects of direct racism on adults health outcomes, but there is limited data on the impacts of vicarious racism on adolescents and their emotional health. Moreover, there is even more limited data on successful strategies to help minorities combat the negative effects of racism in their lives. Objective: This study seeks to examine how adolescents interact with social media and racially-charged stories online. We then explore the emotional responses and strategies adolescents utilize to combat those emotions. Methods: We conducted four (N=4) focus groups with adolescents aged 13-19, with a total of 18 participants. The research team conducted semi-structured focus groups. Focus groups were recorded, transcribed, coded, and analyzed. Results: This paper focused on three main themes that emerged from the data: helplessness, resilience/support systems, and activism as a coping strategy. Adolescents mentioned feelings of helplessness as an initial reaction to racist events and a sense of lack of control. On the other hand, they were able to identify people in their lives who they lean on for support and how activism can sometimes combat those feelings and help them feel like they are making a difference. Discussion: Adolescents are astutely aware of racial conflicts. Not only are they able to properly identify the conflicts, but they have the nuanced view of how racial tensions affect them and people around them. The insights gained from this focus group may allow stakeholders to consider anti-racism strategies that include addressing adolescents emotional needs, including the lack of control, and developing coping strategies on familiar platforms, like social media.

- The National Center for Data to Health (CD2H) was established through a 5-year award from the National Center for Advancing Translational Sciences (NCATS) and will coalesce and coordinate informatics activities across the Clinical and Translational Science Award (CTSA) Program, a network of more than 50 medical research institutions, to provide collaborative clinical and translational research infrastructure. The CD2H supports a vibrant and evolving collaborative informatics ecosystem for the CTSA Program and beyond. Collaborative innovation also serves as a strong foundation to support mechanisms to facilitate training, engagement, scholarly dissemination, and impact in translational science.

Conclusions: The insights gained through evaluation, in particular using publication analysis, allow both investigators and the institutions to convey the benefits and impact of their research and clinical efforts to stakeholders. Publication analysis also provides valuable information that can be used for benchmarking, forecasting, and strategic planning activities.

- The Influence of Acculturation on Latino Adults Beliefs, Attitudes, and Behaviors Toward Tobacco Use: Results of 2014 Focus Groups
- Abstract:
- Objective To explore the influence of acculturation on the beliefs, attitudes, and behaviors toward tobacco use in Latino adults.Methods Data from twenty-four focus groups conducted at four different locations (Chicago, San Diego, Miami, Bronx-New York) were transcribed and uploaded to Atlas.ti. Groups were classified as either high acculturation or low acculturation based on the language spoken during the focus group. English groups were high acculturation while Spanish groups were low acculturation. Mentions of tobacco use in the community and family, tobacco product awareness, tobacco cessation resources, tobacco-related health effects, sources of health information, and second-hand smoke were noted based on acculturation level. Once the review was complete, information collected was compared between acculturation level groups. Results There were no representative differences between English and Spanish-speaking groups in reference to tobacco product use, tobacco product knowledge, and family influence in starting to use tobacco products. Major differences between groups were seen with English-speaking groups who were more likely to show less perceived risk in regard to tobacco use and second-hand smoke being a problem in the community, while Spanish-speaking groups were more likely to show high-perceived risk related to second-hand smoke and health effects associated to tobacco use.

- The impact of SAILS Workforce Development Program on perception of job-specific competency and global self-esteem for youth with chronic illnesses

- Objective:To measure the impact of the SAILS (Supporting Adolescents with Independent Life Skills) WorkforceDevelopment Program on the perception of job competency and global self-esteem of the participants living with a chronic illness or illnesses. Methods: Previous Lurie Childrens Hospital (LCH) patients, aged 18-24, participated in the SAILS Workforce Development program from 2016 to 2017 were selected for this study. The program is a 120 hour paid work experience at a department within the hospital. Data was collected by the LCH transition team, where patients were administered paper-based self-reported surveys both pre- and post- program.The Susan Harter Self-Perception Profile for Emerging Adults was used for the intake and the exit survey to measure self-esteem and perceived job competence,and the Stanford SPS-6 Presentism Scale was used for the exit survey to measure presenteeism. The total scores of the Susan Harter Self-Perception Profile for emerging Adults pre- and post programs were compared using a two-tailed paired t-test.Results: A total of 12 participant data was reviewed and analyzed. The SAILS program may have had a positive effect on the participant's ability to manage their medication and treatment plan. It appears that more than average job days are missed for these youth with a chronic illness. In both the "Job or occupational competence" and "Global self-esteem" subscales of the Susan Harter Self-Perception Profile, the post-program survey scored higher compared to the pre-program survey, however, the findings did not reach statistical significance. The findings also demonstrate high scores on the Presenteeism Scale.Discussion: Despite the modest sample size, results suggested a positive impact on the program participants' general self-esteem and presenteeism. This indicates the beneficial impact of the SAILS Workforce Development program itself. Further research focusing on various types of chronic illnesses that affect youth in specific ways is needed to understand the disease-specific factors and its impact on work.

- Abstract:
- Impaired blood flow due to abnormal rheologic characteristics results in a multiplicity of clinical manifestations, collectively termed the hyperviscosity syndrome. A basic knowledge of the principles of rheology is important in the understanding of its pathophysiology, especially the relationship between viscosity and flow conditions. The flow characteristics in different types of blood vessels are also determinants in the location of the clinical manifestation. The syndrome can occur in a wide variety of diseases and is best grouped according to the causative element or elements in blood. Abnormalities in the cellular components of blood can occur in the quantity and the quality of erythrocytes, leukocytes, and platelets. Abnormal plasma components can also be in both the quantity and quality of the plasma proteins. Clinical manifestations are the result of vascular occlusion, especially in the microcirculation. The altered rheologic characteristics of either the cellular or the protein component may be temperature dependent, being abnormal only at temperatures below 37 degrees C, so that only the cooler parts of the body are affected. The management of these conditions should be primarily directed at the removal of the abnormal component. At the same time, it should be accompanied by measures that can control the production of the causative element.

- The Hidden Burden of Liver-Related Mortality in Patients with Cirrhosis and Low MELD score
- Abstract:
- A poster presented at the 2018 meeting of the American Association for the Study of Liver Diseases.The MELD score has provided the basis for organ allocation in liver transplantation for nearly two decades and is reliable in predicting 90 day mortality in patients with higher scores. However, it is unclear which patients with low MELD-Na (<15) have a higher than expected liver-related mortality and thus would benefit from liver transplantation. We sought to quantify the burden of liver-related mortality in the overall low MELD population as most studies have utilized national transplant databases which only include patients referred for transplantation,. Methods: We utilized HealthLNK, a Chicago-wide database, which incorporates the de-identified electronic medical records from six health care institutions: five large academic medical centers (Northwestern Medicine, University of Chicago Hospitals and Clinic, Rush University Medical Center, University of Illinois at Chicago Medical Center, and Loyola University Medical Center) and one large academic safety net health care system (Cook County Health and Hospitals System). We included adult patients with icd-9 codes of cirrhosis whose MELD-Na scores were never higher than 15 and who died during the study period between January 1, 2006, and December 31, 2012. Records were linked with the state wide death registry and causes of death based on death certificate were attributed to liver related, non-liver related, and non-descript adjudicated by a transplant surgeon and two hepatologists. Results: A total of 20,122 patients with cirrhosis were captured during the above time period. Of these patients, 7,834 had a usable MELD-Na score with 3,717 under 15 for the entire time period. Of this low-MELD-Na subgroup there were 494 deaths (13.3% mortality). Causes of death in this group were 163 (32.9%) Liver related, 144 (29.0%) Non-Liver related, 85 (17.1%) Non-Descript and 104 (21%) missing. Within the Liver related group, 42 died of infection, 17 died of hepatocellular carcinoma, 15 died of portal hypertensive complications, 10 patients died of bleeding and the remaining 79 patients did not have enough detail to attribute to a sub-category. Conclusion: Despite persistently low MELD during a six year time period, 1/3 of patients deaths were attributable to liver disease. Future research should aim at predicting those with a high risk of liver-related mortality in the low MELD population, such that they might benefit from referral and early liver transplantation to reduce this risk.

- The Fate of Obstetric Fistula: Towards Equity, Education, and Policy in the Sylhet Region of Bangladesh
- Abstract:
- This project will describe a review of literature and recommendations for obstetric fistula prevention programming and policy in the Sylhet region of Bangladesh. In order to better understand the social implications that women suffering from obstetric fistula face, an analysis of the interpersonal health communication topics of stigma, disclosure, coping, and identity will be conducted. Obstetric fistulas are an issue of health equity for women in resource-poor settings across the globe. An obstetric fistula is a hole that forms between the vagina and rectum or bladder as a result of prolonged obstructed labor, leaving a woman incontinent of urine, feces, or both. It is estimated that more than two million women currently live with untreated obstetric fistulas in sub-Saharan Africa and Asia (countries with the highest prevalence). The Sylhet region of Bangladesh has been disproportionately impacted in comparison to other regions. There are currently 70,000 women in need of fistula repair surgery and approximately 2,000 new cases of fistula develop every year. Only about 40% of births are attended by skilled personnel and the lifetime risk of maternal death is approximately 1 in 240. Programming recommendations include raising awareness on fistula prevention by training skilled personnel on the signs/symptoms and treatment options. Training community health workers on access, signs, and symptoms of obstructed labor provides an opportunity for depth and breadth of community-based prevention efforts. Training lay individuals to become skilled birth attendants is essential given the lack of midwives and health professionals in rural areas. Health and education policy should focus on training and access for all women at national and regional levels.

- The clinical epidemiology of allergic contact dermatitis
- Abstract:
- Background: Allergic contact dermatitis (ACD) is a common dermatologic diagnosis affecting over 13million patients annually in the United States. Despite its prevalence, limited research has been conducted regarding its clinical epidemiology. The development of contact dermatitis can vary based on genetics, environmental exposures, and co-morbidities such as atopic dermatitis (AD). This project examined the relationship of sex, race, age, and socioeconomic status with the risk of developing allergic contact dermatitis. A secondary goal of this project was to determine the predictors of and relevant allergens in ACD among patients with AD.Materials: We performed a retrospective chart review of 395 adults who were patch-tested at the Northwestern Medicine patch-testing clinic from 2014-2017. Patients were patch-tested with the North American Contact Dermatitis Group (NACDG) standard series and a supplemental allergen series. Demographic data such as sex, age, race, insurance,birthplace, and zip code (as a surrogate for income) were collected. Chi square tests and multivariable logistic regression models were used to estimate adjusted odds ratio and 95% confidence intervals.Results: There were no associations between sex, age, race, insurance, birthplace and the development of ACD. However, individuals estimated to be making less than the median income had an increased risk of developing allergic contact dermatitis (adjustedodds ratio [95% confidence interval] (3.50 [1.36 9.02]). AD patients (n=97) had significantly higher rates of positive patch test reactions to ingredients in their personal care products or topical medicaments, including lanolin (P=0.03), quaternium-15 (P=0.04), fragrance mix I (P=0.008), cinnamal (P=0.02), neomycin (P=0.02), bacitracin (P=0.04), chlorhexidine (P=0.04), and budesonide (P=0.01).Conclusions: Low income was the only demographic factor found to be associated with the development of ACD. Patients with AD did not have higher rates of positive patch test reactions overall. However, they had higher rates of positive patch test reactions tomultiple ingredients in their personal care products and topical steroid and antibiotic medicaments. Future research is needed to understand the risk factors associated with ACD to better predict and prevent this burdensome disease.

- Background: As children with disabilities approach participation, they encounter many challenges. Little has been done to understand the facilitators and barriers to social engagements in the daily lives of children and their families. Purpose: This investigation examines ethnographic data to understand the role of stigma in the development and social participation of African Americanchildren with disabilities and their families. Methods: For the present analysis, a subset of data has been drawn from a 15-year, three-phase, longitudinal ethnographic study collectively titled Boundary Crossings, a study of African American children with special health care needs, their families, and the practitioners who provided services.Results: In the observation of daily life experiences for children with special needs and their families, themes of marginalization based on unique qualities and abilities of the child and family emerged across families stories. While both individual and societal influences are apparent in this data, this analysis focuses on the joint actions and interactions between mothers and their children. Many families in this study narrated experiences of anticipating and/or encountering harsh criticisms and marginalization in social contexts. In some families, stigma was co-managed between the parents and the children. In others, children more autonomously attempted to manage constraints on participation. Implications related to identity, sense of self, stigma, and social engagements are discussed. Conclusion: Though stigma was not a term used by the families themselves, data analysis revealed ways the families understood and managed stigma. Profound examples emerged of how stigma affects family roles and how family roles affect stigma.

- The Association Between Timely Access to Pediatric Appendicitis Care and Neighborhood Factors
- Abstract:
- BACKGROUND: Appendicitis is the most common acute pediatric surgical condition, with 30% of patients presenting as complicated. Complicated appendicitis may indicate a delay in seeking care, resulting in higher complication rates, longer lengths of stay and higher readmission rates compared to simple appendicitis and can serve as indicator for access to care. Although patient-level and neighborhood-level factors can affect timely access to care, the effect of neighborhood factors on access to pediatric surgical care remains poorly understood. We examined the association between neighborhood social determinants of health and the odds of presenting with complicated appendicitis and unplanned post-discharge healthcare utilization. METHODS: A retrospective, cohort study of pediatric patients with appendicitis between 2016-2018 were identified using the Pediatric Health Information System (PHIS) database. Neighborhood characteristics were captured using the Child Opportunity Index (COI), a validated, comprehensive measure of 29 neighborhood characteristics known to impact childrens health. Nationally-normalized COI measurements were divided into quintiles from very low to very high opportunity. Hierarchical logistic regression was used to model the odds of presenting with complicated appendicitis as a function of COI. Adjustments included age, sex, race/ethnicity and insurance. A similar model was assessed for unplanned healthcare utilization, including emergency department visits and readmissions. RESULTS: A total of 67,489 patients had appendicitis with 21,728 (32.2%) being complicated. Patients were 43.3% non-Hispanic white, 60.1% male, 47.8% publicly-insured, and predominantly aged 10-14 years (42.7%). Patients living in very low COI neighborhoods were 34% more likely to present with complicated appendicitis (OR 1.34, 95%CI 1.26, 1.42) compared to those in very high COI neighborhoods. There was no significant association between COI level and unplanned post-discharge healthcare utilization. CONCLUSIONS: Children living in lower COI neighborhoods had an increased risk of presenting with complicated appendicitis; however, neighborhood characteristics were not associated with unplanned healthcare utilization post-discharge. Given increased interest among health systems, public health organizations and third-party payers in mitigating the effects of disadvantaged neighborhood characteristics on health outcomes, these findings may inform policies and programs that seek to improve equitable outcomes in pediatric surgical care.

- The Association between Psychosocial Factors and Pro-Inflammatory Cytokines (IL-6 and IL-8) in Colorectal Cancer Patients
- Abstract:
- Background: Colorectal cancer(CRC) is the third most common cancer among both men and women, and progression of CRC has been linked to pro-inflammatory states. Psychological factors, such as loneliness and poor social support, have been associated within creased serum levels of pro-inflammatory cytokines in a variety cancer cell types. Methods: As part of the Can Support pilot study, participants with newly diagnosed CRC provided blood samples and completed the PROMIS short form survey measures for social isolation, emotional support, companionship, informational support, and instrumental support prior to undergoing surgery. Serum IL-6 and IL-8 levels were determined via ELISA. Correlation as well as simple and multiple linear regression were used to assess the relationship between the serum cytokine levels and psychosocial factors.Results: From a total of 216 screened patients, complete data were obtained from 39 patients. The average age at diagnosis was 57.9years, and 51.3% were women. Due to protocol deviations, a total of 6 patients had blood drawn while in the OR as opposed to during standard pre-operative office visits. Average IL-6values trended towards significantly differing with respect to draw-time (p < 0.087). None of the five psychosocial variables correlated with normalized values of serum IL-6 or IL-8values. Simple linear regression models were created for each psychosocial and bio-marker pair, and none of the psychosocial variables significantly explained or correlated with either IL-6 or IL-8. Multivariate analysis with relevant covariates was completed for each psychosocial and bio-marker pair. Only instrumental support was identified as an independent predictor of IL-8 serum levels when adjusting for various covariates.Conclusion: There was little to no observed association between various social support measures and serum IL-6 or IL-8 levels, but the results do help to highlight the importance of protocol design, namely blood collection time, which helps to inform future trial design.
