- Adverse Childhood Experiences (ACEs) among reproductive age women: Findings from the 2019 BRFSS
- Abstract:
- Background and Significance: The 1998 Adverse Childhood Experiences ACE Study defines ACEs as ten categories of abuse, neglect, and family stressors before age 18. ACEs contribute to poor health and wellbeing for all genders across the lifespan. Despite rising premature mortality among women, and the impact of childhood adversity on girls and women, scant research examines trends in ACEs among women of reproductive age. Methods: This study utilizes the 2019 BRFSS Survey and focuses on women and men ages 18-39 in 17 states. The study first analyses gender differences in ACE score and specific ACE exposures. The study then examines age differences in ACE scores between four age cohorts of women of reproductive age (18-24, 25-29, 30-34, 35-39 years). Finally, the study analyses associations between womens ACEs scores and adult physical and mental health, self-reported BMI categories, history of depression, and fair to poor overall health by age cohort. Results: Findings revealed a higher prevalence of 4+ ACE scores among women as compared to men ages 18-39 years (5.42% difference). A greater proportion of women experienced sexual abuse (16.49% vs. 5.91%), lived with someone experiencing mental illness (22.16% vs. 15.44%), with alcohol abuse (22.31% vs. 18.32%) and drug/prescription medication abuse (13.58% vs.12.16%), and witnessed domestic violence at home (16.59% vs. 12.84%) compared to men. Younger reproductive age women (18-24 years) experienced a greater prevalence of 4+ ACEs (24.67%) as compared to older reproductive age women (35-39 years) (20.06%; 4.61% difference). In contrast to those without a history of ACEs, women with 4+ ACEs were 1.59 (95% CI, 1.41-1.78) times as likely to develop obesity, 3.46 (95% CI 3.07-3.90) times as likely to have a history of depression, and 1.68 (95% CI 1.45-1.94) times as likely to report fair or poor health. Discussion: ACEs are associated with significant health problems among reproductive age women. These findings provide a critical foundation for future social epidemiology research focused on this population. Awareness of the prevalence and impact of ACEs must be included in public health planning, policy, and practice to improve the health of reproductive age women.

- Background: Adverse childhood experiences (ACEs) are stressful or traumatic events experienced in childhood and are related to the development of a broad range of health problems throughout a person's lifespan. Reporting four or more ACEs is strongly associated with adult health and social problems, however, there is a lack of research studying individuals ability to overcome ACEs and achieve successful outcomes. The purpose of this study was to examine the proportion of individuals who reported four or more ACEs who attained 'successful' health and economic outcomes as adults.Methods: Data were obtained from the Centers' for Disease Control and Prevention's 2011-2012 Behavioral Risk Factor Surveillance System (BRFSS) from nine states with respondents to the ACEs question module (N=86,968), and were presented as weighted estimates to be representative of the approximate 32 million adult state residents. 'Successful' adult health was defined as excellent, very good, or good self-reported health and economic 'success' was defined as being in the upper 50% of household income in an age group. The likelihood of health and economic 'success' across four categories of reported ACEs are reported.Results: Comparing respondents who reported zero ACEs to those reporting four or more, 93.5% versus 84.1% of age 18-34 attained successful health outcomes, respectively, and 56.3% versus 44.9% attained successful economic outcomes. Of respondents age 35-54, 86.3% and 49.7% with an ACE count of zero attained successful health and successful economic outcomes, respectively, compared to 74.5% and 38.2% of respondents with an ACE count of four or more. Among respondents age 55 and over, 77.0% and 58.5% with an ACE count of zero attainedsuccessful health and successful economic outcomes, respectively, compared to 64.9% and 59.2% of respondents with an ACE count of four or more, all comparisons p <0.001.Conclusion: Study findings indicate those with four or more ACEs attain significantly fewer 'successful' health and economic outcomes compared to those with zero ACEs, however, a majority of those with high ACE counts were successful under the definitions used in the study. More research is needed on patterns of resilience among those who have successfully coped with ACEs.

- Abstract:
- The adverse effects of drugs used in the treatment of hematologic malignancies are among the many factors contributing to the increased risk of both thrombosis and bleeding. These effects most often occur when combination of drugs are given. Some, such as L-asparaginase, result in both bleeding and thrombosis. Consideration must be given also to the bleeding or prothrombotic risk of the underlying hematologic disorder. The commonly used drugs with adverse effects on hemostasis include L-asparaginase, corticosteroids, inhibitors of vascular endothelial growth factor, gemtuzumab ozogamicin, thalidomide, and immunomodulatory derivatives of Thalidomide, and the hematopoietic growth factors. In addition, the syndrome of thrombotic microangiopathy may be brought on by several other drugs. Thus, a full understanding of these adverse effects is necessary in treating these disorders.
