- Muscle activation helps stabilize the glenohumeral joint and prevent dislocations, which are more common at the shoulder than at any other human joint. Feedforward control of shoulder muscles is important for protecting the glenohumeral joint from harm caused by anticipated external perturbations. However, dislocations are frequently caused by unexpected perturbations for which feedback control is essential. Stretch-evoked reflexes elicited by translations of the glenohumeral joint may therefore be an important mechanism for maintaining joint integrity, yet little is known about them. Specifically, reflexes elicited by glenohumeral translations have only been studied under passive conditions, and there have been no investigations of how responses are coordinated across the functional groupings of muscles found at the shoulder. Our objective was to characterize stretch-evoked reflexes elicited by translations of the glenohumeral joint while shoulder muscles are active. We aimed to determine how these responses differ between the rotator cuff muscles, which are essential for maintaining glenohumeral stability, and the primary shoulder movers, which are essential for the large mobility of this joint. We evoked reflexes using anterior and posterior translations of the humeral head while participants produced voluntary isometric torque in six directions spanning the three rotational degrees-of-freedom about the shoulder. Electromyograms were used to measure the stretch-evoked reflexes elicited in nine shoulder muscles. We found that reflex amplitudes were larger in the rotator cuff muscles than in the primary shoulder movers, in part due to increased background activation during torque generation but more so due to an increased scaling of reflex responses with background activation. The reflexes we observed likely arose from the diversity of proprioceptors within the muscles and in the passive structures surrounding the shoulder. The large reflexes observed in the rotator cuff muscles suggest that feedback control of the rotator cuff augments the feedforward control that serves to compress the humeral head into the glenoid. This coordination may serve to stabilize the shoulder rapidly when preparing for and responding to unexpected disturbances.

- Towards semi-automatic monitoring of delivery behavioral interventions
- Abstract:
- Traditional methods for monitoring implementation of evidence-based programs require labor-intensive quality assessments. These assessments generally involve human observation, hence becoming a major bottleneck in the monitoring of implementation. We present the development of computer-based methods for implementation quality measurement. Firstly, we describe 7 principles of automatization necessary tosystematically identify critical dimensions for analysis.Secondly, we present an automatic classifier that uses linguistic patterns to classify sessions between high-fidelity versus low-quality. The classifier was trained on an initial corpus of 22 transcripts which were coded for high and low quality by humans and replicated by machine learning. Our goal is to develop efficient methods that reduce the burden of quality monitoring, and provide timely feedback for delivery supervisors.Overall, we want to explore to what extent quality monitoring can be done with automatic methods [1,3,4].

- Abstract:
- This article was motivated by the conference entitled "Perception & Action - An Interdisciplinary Approach to Cognitive Systems Theory," which took place September 14-16, 2010 at the Santa Fe Institute, NM, USA. The goal of the conference was to bring together an interdisciplinary group of neuroscientists, roboticists, and theorists to discuss the extent and implications of action-perception integration in the brain. The motivation for the conference was the realization that it is a widespread approach in biological, theoretical, and computational neuroscience to investigate sensory and motor function of the brain in isolation from one another, while at the same time, it is generally appreciated that sensory and motor processing cannot be fully separated. Our article summarizes the key findings of the conference, provides a hypothetical model that integrates the major themes and concepts presented at the conference, and concludes with a perspective on future challenges in the field.

- Abstract:
- Environmental surveillance is a critical tool for combatting public health threats represented by the global COVID-19 pandemic and the continuous increase of antibiotic resistance in pathogens. With its power to detect entire microbial communities, metagenomics-based methods stand out in addressing the need. However, several hurdles remain to be overcome in order to generate actionable interpretations from metagenomic sequencing data for infection prevention. Conceptually and technically, we focus on viability assessment, taxonomic resolution, and quantitative metagenomics, and discuss their current advancements, necessary precautions and directions to further development. We highlight the importance of building solid conceptual frameworks and identifying rational limits to facilitate the application of techniques. We also propose the usage of internal standards as a promising approach to overcome analytical bottlenecks introduced by low biomass samples and the inherent lack of quantitation in metagenomics. Taken together, we hope this perspective will contribute to bringing accurate and consistent metagenomics-based environmental surveillance to the ground.

- Thienopyridine derivatives (ticlopidine, clopidogrel, and prasugrel) are the primary antiplatelet agents. Thrombotic thrombocytopenic purpura (TTP) is a rare drug-associated syndrome, with the thienopyridines being the most common drugs implicated in this syndrome. We reviewed 20 years of information on clinical, epidemiologic, and laboratory findings for thienopyridine-associated TTP. Four, 11, and 11 cases of thienopyridine- associated TTP were reported in the first year of marketing of ticlopidine (1989), clopidogrel (1998), and prasugrel (2010), respectively. As of 2011, the FDA received reports of 97 ticlopidine-, 197 clopidogrel-, and 14 prasugrel-associated TTP cases. Severe deficiency of ADAMTS-13 (a disintegrin and metalloproteinase with a thrombospondin type 1 motif member 13) was present in 80% and antibodies to 100% of these TTP patients on ticlopidine, 0% of the patients with clopidogrel-associated TTP (p < 0.05), and an unknown percentage of patients with prasugrel-associated TTP. TTP is associated with use of each of the three thienopyridines, although the mechanistic pathways may differ.
