- Abstract:
- Uterine leiomyomas (fibroids) are a major source of gynaecologic morbidity in reproductive age women and are characterised by the excessive deposition of a disorganised extracellular matrix, resulting in rigid benign tumours. Clinically, leiomyoma patients usually present with pelvic pain, urinary incontinence, as well as heavy cyclic and non-cyclic bleeding. Curative treatment options are limited, with hysterectomies currently the only guaranteed treatment to prevent reoccurrence.Uterine fibroid tumours are composed primarily of smooth muscle cells and fibroblasts. They have also been shown to be clonal, with multiple tumours from the same patient sometimes originating from different tumour initiating cells. Leiomyomas fall into four main mutational subtypes: tumours with genetic aberrations in either fumarate hydratase (FH), mediator of transcription subunit 12 (MED12), high mobility group AT-hook 2 (HMGA2), or type IV collagens. Somatic mutations in Mediator complex subunit MED12 have been implicated as the causal genetic lesion in the majority of leiomyoma. The functional consequence of this mutation with respect to altered epigenetic events and subsequent transcriptional dysregulation remains poorly understood.Epigenetic control of gene expression by promoter-distal cis-regulatory elements such as enhancers is an established and important mechanism of gene regulation, with aberrant epigenetic changes at distal sites being implicated in human disease. Despite this, the role of enhancer regulation in uterine leiomyomas remains unexplored. In this study, optimised extraction procedures for fibrotic tissues, coupled with high resolution ChIP-sequencing, promoter capture Hi-C, and RNA-sequencing of matched normal myometrium and diseased leiomyoma tissues, are used to provide an integrative analysis of transcriptional dysregulation mechanisms in leiomyomas. Through the characterisation of the first chromatin interactome in uterine leiomyomas, this study demonstrates that modified enhancer architecture is an important mechanism of transcriptional dysregulation in uterine fibroids.Activator protein 1 (AP-1) is a dimeric transcription factor that responds to varied stimuli and is responsible for the regulation of many cellular processes such as proliferation and differentiation. AP-1 subunits belonging to JUN, FOS and ATF gene families have previously been demonstrated to be down regulated in uterine leiomyomas. This study highlights the link between decreased AP-1 subunit gene expression and modifications in leiomyoma tissue enhancer architecture. The loss of AP-1 expression results in the depletion of AP-1 occupancy on chromatin in leiomyoma tissue samples, which correlates with changes in H3K27 acetylation at distal cis-regulatory elements.Importantly, this study also demonstrates that silencing of AP-1 subunits in primary human uterine smooth muscle cells leads to large-scale transcriptional dysregulation. In addition, loss of AP-1 causes significant alteration to the H3K27Ac cistrome in uterine smooth muscle cells, primarily at promoter-distal sites, thereby partially recapitulating epigenetic changes seen in leiomyoma tissue samples. The data supports the hypothesis that AP-1 plays an important mechanistic role in enhancer maintenance and the loss of AP-1 results in altered enhancer architecture in uterine smooth muscle cells. These findings establish AP-1 driven aberrant enhancer regulation as an important mechanism of leiomyoma disease pathogenesis.

- Abstract:
- This study characterized center-of-mass (CoM) dynamics of below-knee prosthesis users and non-impaired controls following a lateral perturbation when the perturbation direction is known but a priori knowledge of the timing of perturbation is either known or unknown. Previous research found that below-knee prosthesis users proactively increase their lateral margin-of-stability on their impaired side in anticipation of an impending perturbation when the timing is predictable and potentially directed toward the impaired limb. While knowledge of perturbation timing and direction influences proactive strategies, the consequences of such knowledge and anticipatory behavior on recovery from perturbations is unclear. This study characterized center-of-mass (CoM) dynamics of below-knee prosthesis users and non-impaired controls following a lateral perturbation when the perturbation direction is known but a priori knowledge of the timing of perturbation is either known or unknown. Across groups, CoM displacement during perturbation exposure increased when directed towards the impaired or non-dominant limb with no influence of timing knowledge. In addition, peak CoM displacement was less with known timing irrespective of the perturbation direction. Generally, the CoM displacement during perturbation exposure correlated well with the CoM medial-lateral velocity during unperturbed walking, supporting evidence that human response dynamics to lateral perturbations is influenced by the instantaneous state of the bodys momentum.

- Abstract:
- It is well recognized that cancer patients have an increased risk of thrombosis and bleeding. Contributory factors to thrombotic complications include prothrombotic risks carried by many chemotherapeutic agents, type of cancer, stage of cancer, age, and comorbidities. Surgical procedures and the presence of an indwelling vascular device, often used as a mode of delivery for chemotherapy, further increase the risk. Correlative studies have demonstrated upregulation of coagulation in response to chemotherapeutics, and clinical studies have shown that various cancer treatments are independent risk factors for thrombotic complications. It has also been shown that patients who develop thrombosis during treatment have a worse overall prognosis. Mechanisms for chemotherapy and cancer-associated thrombosis are not well understood. Better insight into the mechanism for thrombosis may help better identify those cancer patients at highest risk, who may then benefit from up-front anticoagulant prophylaxis.

- Therapeutic apheresis is an extracorporeal blood purification technique designed for the removal of either plasma (plasmapheresis) or cellular blood components (cytapheresis). One of the main indications for the use of apheresis is in the treatment of the hyperviscosity syndromes that can result from either the presence of abnormal plasma components, such as antibodies, immune complexes, paraproteins, and cryoglobulins, or the excessive increase in blood cells as seen in polycythemia, leukemias, and myeloproliferative diseases. Apheresis involves withdrawal of anticoagulated blood via a vascular catheter, separation of different blood components by either centrifugation or membrane filtration, removal of the undesired component, and reinfusion of the remaining components with replacement fluid into the patient. The centrifugal method can be intermittent or continuous, the latter being faster and fully automated, and is principally used in North America. The membrane filtration technique, mainly used in Europe and Japan, involves the filtration of blood by filters of different pore sizes. These are used sequentially in a process called double or cascade filtration, enabling removal of specific plasma pathogens without need for replacement fluids. In paraproteinemias, hyperviscosity syndrome is most commonly seen with Waldenstrm's macroglobulinemia, followed by immunoglobulin (Ig) A and IgG (3) multiple myeloma. Single plasmapheresis with one plasma volume replacement (about 3 L) usually results in a dramatic improvement in patients with macrogobulinemia because of its predominant intravascular distribution, whereas repeated plasmapheresis is necessary with other types of paraproteins. Cryofiltration apheresis using a high-capacity cryofilter is specific for the removal of cryoglobulins. In leukemias with hyperleukocytosis, there are no evidence-based guidelines for use of leukapheresis, but it is commonly initiated when white blood cells (WBC) are > 100,000/microL or even with lower counts if leukostasis symptoms are present, especially in acute myeloid leukemia. Erythrocytapheresis and plateletpheresis are mostly used in the acute management of symptomatic patients with polycythemia vera (PV), essential thrombocytosis, and sickle cell disease.

- Twelve evidence-based profiles of roles across the translational workforce and two patients were made available through CTS Personas, a project of the CTSA Program National Center for Data to Health (CD2H). The persona profiles were designed and researched to demonstrate the key responsibilities, motivators, goals, software use, pain points, and professional development needs of those working across the spectrum of translation, from basic science to clinical research to public health. The projects goal was to provide reliable documents that could be used to inform CTSA software development projects, educational resources, and communication initiatives.This paper presents the initiative to create personas for the translational workforce, including the methodology, engagement strategy, and lessons learned. Challenges faced and successes achieved by the project may serve as a roadmap for others searching for best practices in the creation of Persona profiles.

- Background: There is a wide variety of dissemination and implementation designs now being used to evaluate and improve health systems and outcomes. Methods: This paper is one product of a design workgroup formed in 2013 by the National Institutes of Health (NIH) in order to address dissemination and implementation research, and whose members represented diverse methodologic backgrounds, content focus areas, and health sectors. These experts integrated their collective knowledge on implementation designs with searches of published evaluations of implementation strategies. Results: This paper describes designs for the traditional translational pipeline, which builds on existing efficacy and effectiveness trials to examine how an evidence-based clinical/prevention intervention is adopted, scaled up, and sustained in community or service delivery systems. A range of randomized and non-randomized designs is used in this approach that focuses on producing generalizable knowledge about implementation and dissemination.

- The mosaic of translation: an analysis of translational medicine publications

- Abstract:
- Objective: Research impact and evaluation services are incredibly important in the area of translational medicine. One method for evaluation is publication analysis, which plays a valuable role in helping an institution gain insights about its research and clinical activities that might not be discernible from other means, including some of the available institution-level data stores. Methods: We used publication analysis and information visualization to evaluate the outputs of translational medicine. We discussed the results of harvesting bibliographic metadata from literature databases, analyzing percentile rankings of publications, and reviewing topical trends over time. Results: We found that coverage of journals with a translational focus varies by resource. Additionally, each resource allows for differing types and quantities of exportable metadata. Publications were analyzed based on the available metadata, and visualizations were created to show the results of the publication analysis. Conclusions: The insights gained through evaluation, in particular using publication analysis, allow both investigators and the institutions to convey the benefits and impact of their research and clinical efforts to stakeholders. Publication analysis also provides valuable information that can be used for benchmarking, forecasting, and strategic planning activities.

- Abstract:
- Objective: Research impact and evaluation services are an incredibly important in the area of translational medicine. One method for evaluation is publication analysis, which plays a valuable role in helping an institution gain insights about its research and clinical activities that might not be discernible from other means, including some of the available institution-level data stores. Methods: We used publication analysis and information visualization to evaluate the outputs of translational medicine. We discussed the results of harvesting bibliographic metadata from literature databases, analyzing numbers of publications over time, and reviewing topical trends in the literature. Results: We found that coverage of journals with a translational focus varies by resource. Additionally, each resource allows for differing types and quantities of exportable metadata. Publications were analyzed based on the available metadata, and visualizations were created to show the results of the publication analysis. Conclusions: The insights gained through evaluation, in particular using publication analysis, allow both investigators and the institutions to convey the benefits and impact of their research and clinical efforts to stakeholders. Publication analysis also provides valuable information that can be used for benchmarking, forecasting, and strategic planning activities.

- Since the inception of the Clinical and Translational Science Award (CTSA) program in 2006, leaders in education across CTSA sites have been developing and updating core competencies for Clinical and Translational Science (CTS) trainees. By 2009, 14 competency domains, including Biomedical Informatics, had been identified and published. Since that time, the evolution of the CTSA program, changes in the practice of CTS, the rapid adoption of electronic health records (EHRs), the growth of biomedical informatics, the explosion of big data, and the realization that some of the competencies had proven to be difficult to apply in practiceall made it clear that the competencies should be updated. This paper describes the process undertaken and puts forth a new set of competencies that has been recently endorsed by the Clinical Research Informatics Workgroup (CRI-WG) of AMIA. In addition to providing context and background for the current version of the competencies, we hope this will serve as a model for revision of competencies over time.
